Sad, Very SadI'm living with my DD and family. She does not want to talk of this at all, but I bring it up occasionally as it seems necessary. I have a notebook of legal documents, all but one supplied by the state in which I live. Most need notarized, some need only two unrelated witnesses.
Durable Power of Attorney. This one is "springing"--springing into use when my doc says I'm incompetent. I changed it to two doctors. It can't be used until then and ends when I do--or when I change it.
State Advanced Health Care Directive
FIVE WISHES booklet, similar to the state one. It's a legal document in about 40 states, but not in mine. I included it because it goes beyond the states form, getting further into emotional, physical, spiritual needs/wants. Available at some medical offices and on the web.
OUT OF HOSPITAL DNR. Legal here. If I die at the grocery, for instance, hopefully the EMTs will honor it.

POST--Physician's Order for Scope of Treatment.
Some critical medical records
Living Will declaration
Copy of my Last Will and Testament.
I do have someone to help me through my last day, so included
Funeral Planning Declaration which gives the named person permission to plan the services, ceremonies, disposition of my remains (also a state form)
Certificate of bequeathal (to an institution) of my body if they'll take it, and info of crematory if they don't
Also included is a link to the n
Neptune Society's site and some of their checklist for when a person dies.
All of that is in a notebook that she can keep with her to use. Another notebook that stays in the house has the originals of all those documents.
Another book has personal and financial information, phone numbers, etc.
Hope this helps. I think most of these papers should be available on your state's website or your doctor's office, except the 5 Wishes booklet and the Neptune information.

I think several of these papers, if they go with you to the hospital, should be honored even if you have no one to advocate for you. I know my POST, Physician's Order of Scope of Treatment is in my hospital's system, available for the medical personell to see.
