A simple answer to all three questions - the bigger the city the farther you want to be away.
Psychos are kind of like water molecules evaporating of a crazy lake and the farther away you are the lower the density of said 'molecules'. Moving away from the rare problem is always easy.
I use a shovel for poop, which also requires being away from a City.
Likewise for showers - I haven't seen the perfect outdoor setup but solar showers work, as do stove top sponge baths. Baby wipes, from the diaper section of the grocery store, are cheap and along with baby powder can be used to extend the time between showers.
Disability is tough and you should be prepared for your eventual decline. Get on the affordable Housing lists for areas that you like, HUD (Section 8 and Project Specific) as well as USDA Rural Housing. Managing multiple waitlists can be an issue and I would only do a few, and in areas that you have at least a minimal connection with. Likely it will be after the Trump administration before you make it to the top.
