I hear you about destructive kids. Mine didn't have many broken pieces but apparently some thought the holes under the ashtrays in the back were a perfect garbage receptacle. I pulled some really really nasty stuff out of there, glad it's clean now though.
 
I picked up a 6x15 piece of carpet today. The old carpet has some holes and stains in it. There is a rubber padding under the old carpet. I am hoping to reuse the thick rubber padding. I removed all the seats and seltbelts already. What would the best way to seal the floor where the  seat mounting holes are? I can use the rear seatbelt mounting bolts to secure my bed frame.

I plan on building setup similar to Nomadic Fanatic. Wood bed frame with storage underneth.
Twin xl memory foam mattress on the back wall. Cabinet on other side about 12" wide. That leaves me about 12" of walkway between the bed and rear cabinet- countertop. And a cabinet between the bed and drivers seat.

Yeah, lay some 1/2 inch plywood over the carpet (carpet will reduce noise and insulate a bit). Then I would put some linoleum over it. Easy to clean and cheaply bought used off craigslist.

In tearing out a conversion van's flooring, siding, and wall insulation for a friend, I discovered the wood flooring under the carpet was particleboard. It had absorbed every wet thing that got to it, including pet urine. The foam carpet underlayment was also a stinky mess. Out it all went. The insulation behind the wall siding was an inadequate mixture of thin soft foam pieces and fiberglass batts and there were spots with no insulation at all.

We scrubbed everything down and sealed the van floor before starting anew.
 
Crazy. I'm surprised your friend didn't notice the urine smell when he/she bought it.. or was it your friend's pet?

I checked my flooring and it seems fine, there's blueboard type insulation (still need to check R-value) situated above particle board, then a layer of rubber and finally carpet, but I am going to rip out the carpet soon here and replace it with laminate 'wood' flooring. The walls were insulated well, surprisingly enough. I think the conversion van company can make some difference in build quality. I still have the original window sticker and the owners ordered an insulation package for which they paid around $2k extra from Mark III.

Almost everyone I hear from mentions poor insulation on conversions from the factory, so this probably isn't really the norm.
 
@East ~ seemed like once the carpet was up, the underlayments released their odors. The price of the van was so excellent, I don't think the owner was too concerned with this part.

I hope the OP will not find this problem though!
