Odyssey van - simple conversion

So my mom offered to let me borrow her Honda minivan in lieu of other options, so I'll be doing a simple conversion and trying it out, just to see if this is for me. I will also try to do some tent camping when the weather allows and the van feels too small. My plan is to try camping for 2-4 days at a time with this setup. Also, I'm planning to get a medium-sized dog as companion/camping buddy.

Since the van's not mine I won't be doing any modifications that disturb the resale value. I just got the van detailed because it had some mold issues (in the vents), and I'm going to use an ozone machine for good measure.

My plans for the bed are: a split piece of plywood joined by a piano hinge, with a foam mattress that's been scored to bend at the hinge, on top of a few milk crates.

It sounds like my best bet is to make a cardboard cutout, take it to Lowes, and ask them to cut it for me. 
Which plywood thickness? Type of plywood?
